Palliative Care Specialist

About the Role
Providence Connections is seeking a board-certified Nurse Practitioner to join a well-established, trans-disciplinary team providing palliative care to patients and their families in primarily outpatient settings. This role will provide evaluation and management of chronic conditions or serious, life-limiting illnesses. The ideal candidate will have previous palliative, oncology, and/or geriatric care experience.
Key Responsibilities
- Collaborate with physicians, nurses, social workers, and chaplains to provide holistic care
- Provide evaluation and management of chronic conditions or serious, life-limiting illnesses
- Work with a well-established, trans-disciplinary team to provide palliative care to patients and their families
Requirements
- Minimum of one year of post-graduate clinical experience in palliative care, hospice, geriatrics, and/or oncology care
- Board-certified Nurse Practitioner
