Job Summary:

Kaiser Permanente is seeking a skilled and compassionate Registered Nurse to join our Hospice and Palliative Care team. As a Public Health Nurse, you will provide expert care to patients and families in a variety of settings, including private homes, adult foster homes, and skilled nursing facilities.

Key Responsibilities:
  • Conduct thorough assessments of patients' physical, psycho-social, and emotional needs, and develop individualized care plans to address these needs.
  • Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions between settings.
  • Provide education and support to patients, families, and caregivers on hospice and palliative care principles and practices.
  • Participate in case conferences and contribute to the development of patient care plans.
  • Supervise and document Hospice Aide care delivery, and provide emotional and clinical support at the time of death.
  • Partner with agency management to develop and review programs within the Continuing Care Services Department.
Requirements:
  • One year of medical-surgical or ICU nursing experience in an acute care setting, ambulatory nursing, or home health, hospice, or palliative care.
  • Baccalaureate degree in nursing or commensurate knowledge and experience.
  • Registered Nurse License (Washington) within 3 months of hire, or Compact License: Registered Nurse within 3 months of hire.
  • Basic Life Support certification.
