We are seeking a skilled and compassionate Hospice and Palliative Medicine Nurse Practitioner to join our team at Provider Solutions & Development.

Responsibilities
  • Assess and manage patients' physical, emotional, and spiritual needs
  • Develop and implement individualized care plans
  • Collaborate with healthcare teams to ensure comprehensive care
  • Provide education and support to patients and families
  • Document patient progress and outcomes
Requirements

To be successful in this role, you will need:

  • Current licensure as a Nurse Practitioner in the state of Oregon
  • Experience in hospice and palliative care
  • Strong assessment and communication skills
  • Ability to work independently and as part of a team
