Geriatric Care Specialist Opportunity
Providence ElderPlace is seeking a skilled Geriatric Care Specialist to join our interdisciplinary team. As a Geriatric Care Specialist, you will work with a dedicated team to provide comprehensive care to older adults in our community.
Key Responsibilities:
- Manage a small panel of patients at your primary practice
- Collaborate with physicians, nurse care managers, social workers, pharmacists, behaviorists, and other team members
- Provide care in outpatient primary care settings, home visits, and community-based settings
- Participate in palliative care and other geriatric-focused initiatives
Requirements:
- Board Certified or Board Eligible Medical Doctor with or without US residency
- Current US work visa
- Minimum of 1-year experience working with a geriatric population
