Seeking a Board-Certified Nurse Practitioner

Providence Connections Palliative Care is a well-established, trans-disciplinary team providing palliative care to patients and their families in a primarily outpatient setting. We are seeking a board-certified Nurse Practitioner to join our team, providing evaluation and management of chronic conditions or serious, life-limiting illness.

Key Responsibilities
  • Evaluate and manage patients with chronic conditions or serious, life-limiting illness
  • Collaborate with physicians, nurses, social workers, and chaplains to provide comprehensive care
  • Participate in a team-based approach to care, prioritizing patient-centered care and quality of life
  • Develop and implement individualized care plans to address the unique needs of each patient
  • Provide education and support to patients and their families to promote self-management and empowerment
Requirements
  • Board certification in a relevant specialty (e.g., palliative care, hospice, geriatrics, oncology)
  • Minimum of one year of post-graduate clinical experience in palliative care, hospice, geriatrics, and/or oncology care
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team-based environment
  • Strong analytical and problem-solving skills, with the ability to prioritize tasks and manage multiple responsibilities
  • Ability to work in a fast-paced environment, with a high level of flexibility and adaptability
