We are seeking a board-certified Nurse Practitioner to join our team, providing evaluation and management of chronic conditions or serious, life-limiting illness.
Key Responsibilities- Evaluate and manage patients with chronic conditions or serious, life-limiting illness
- Collaborate with physicians, nurses, social workers, and chaplains to provide comprehensive care
- Participate in a team-based approach to care, prioritizing patient-centered care and quality of life
- Develop and implement individualized care plans to address the unique needs of each patient
- Provide education and support to patients and their families to promote self-management and empowerment
- Board certification in a relevant specialty (e.g., palliative care, hospice, geriatrics, oncology)
- Minimum of one year of post-graduate clinical experience in palliative care, hospice, geriatrics, and/or oncology care
- Excellent communication and interpersonal skills, with the ability to work effectively in a team-based environment
- Strong analytical and problem-solving skills, with the ability to prioritize tasks and manage multiple responsibilities
- Ability to work in a fast-paced environment, with a high level of flexibility and adaptability
