Join Our Team as a Housing First Program Coordinator

The Flexible Support Services initiative represents a pioneering approach to care, aimed at overcoming various obstacles to achieving stable housing and fostering recovery. The ideal candidate will possess a deep sense of empathy, an open-minded attitude towards recovery pathways and harm reduction concerning substance use disorders, along with expertise in housing navigation and tenancy sustainability. Extensive experience in evaluating and addressing social determinants of health is essential.

Key Responsibilities:

  • Conduct screenings, assessments, and create comprehensive trauma-informed care plans tailored to the specific needs and treatment preferences of clients.
  • Gather and analyze data related to client outcomes, generating reports and trend analyses as required by agreements with relevant Accountable Care Organizations.
  • Facilitate extensive service coordination through necessary collateral contacts while utilizing pre- and post-tenancy support services within a Housing First framework. Complete assessments for Coordinated Entry and/or DMH Services Authorization as needed.
  • Engage in strategic planning for the Flexible Services Program.
  • Enhance existing partnerships with community organizations to develop more comprehensive support service options.
  • Participate as a member of both internal and external multidisciplinary teams and case conferencing forums as assigned.
  • Maintain current certifications in CPR, First Aid, and other required training, ensuring all documentation is up to date.

Qualifications:

  • A Bachelor's degree in social work, counseling, psychology, nursing, or a closely related field is required, or a minimum of 2 years of case management experience with the homeless population.
  • A Master's degree in a relevant field is highly preferred.
  • Recovery Coaches or Certified Peer Specialists with at least one year of experience working with the adult homeless population will be considered.
  • Strong skills in engagement, support planning, implementation, and organization are essential.
  • Proven ability to leverage both internal and external care continuums and resources to meet the identified needs of clients while maximizing their involvement and participation.
  • Knowledge and experience in supporting homeless adults with mental health issues and/or substance use disorders are required, along with familiarity with Public Entitlements and other resources.
  • Maintain an understanding of community resources, particularly housing systems.
  • Possess a working knowledge of housing navigation and tenancy support within a Housing First framework.

Schedule: Monday - Friday, 9 AM - 5 PM.

Salary: Bachelor's degree: $48,000; Master's degree: $50,000.

Bilingual candidates are encouraged to apply and may qualify for a salary increase upon hiring.
