POSITION SUMMARY:

The PrEP Coordinator plays a crucial role in supporting the implementation of the PrEP program. This position involves assisting participants in accessing PrEP, ensuring adherence to medication, and facilitating follow-up care. The coordinator will work collaboratively to deliver HIV and HCV test results, engage in outreach activities, and participate in promotional campaigns aimed at increasing awareness and access to PrEP.

The coordinator will focus on educating individuals who test negative about safe practices and the benefits of PrEP, ensuring they receive necessary lab work and financial assistance for medication. The role includes expanding the low-threshold HIV Pre-Exposure Prophylaxis (PrEP) program at the Puerto Rican Cultural Center, providing intensive care coordination, and helping clients navigate health-related social services.

KEY RESPONSIBILITIES:

  • Conduct confidential assessments to identify the HIV prevention needs of participants, whether their test results are positive or negative.
  • Develop and maintain a network of PrEP/PEP resources and providers in the community.
  • Facilitate access to biomedical prevention services, ensuring adherence and follow-up support.
  • Provide referrals for STI screenings and ensure participants receive appropriate services.
  • Collaborate with project staff to promote the HIV testing program through various media channels.
  • Conduct on-site HIV antibody testing and engage in community outreach.
  • Deliver test results and offer partner services to individuals who test positive for HIV.
  • Perform follow-up assessments to reinforce risk awareness among participants.
  • Coordinate with Disease Intervention Specialists for effective linkage to prevention services.
  • Document program activities and report outcomes as required.
  • Participate in ongoing training and professional development opportunities.
  • Engage in community events and organizational activities to promote health initiatives.
  • Complete additional duties as assigned.

This position requires adherence to all organizational standards, policies, and regulatory compliance requirements.

QUALIFICATIONS:

  • Minimum of two years of experience in patient navigation, case management, or related fields.
  • Experience in HIV/AIDS services is preferred.
  • Strong knowledge of community health resources and social service agencies.
  • Experience working with diverse populations, including LGBTQ, African American, and Latino communities.
  • Familiarity with HIV prevention interventions and STIs.
  • Ability to work effectively with high-risk populations.
  • Knowledge of HIPAA regulations and disease communication.
  • Comfortable in clinical settings with healthcare professionals.

SKILLS:

  • Bilingual in English and Spanish is preferred.
  • Excellent communication and interpersonal skills.
  • Ability to build relationships with individuals from diverse backgrounds.
  • Proficient in computer applications and effective phone communication.
  • Strong multitasking abilities and attention to detail.
  • Knowledge of PrEP and the ability to educate others about it.

EDUCATION:

  • High School Diploma or GED required.
  • Bachelor's degree in social work, public health, or a related field is preferred.

WORKING CONDITIONS:

This position may involve occasional exposure to blood or body fluids. The employee may need to stand or sit for extended periods and occasionally lift up to 20 pounds.
