POSITION SUMMARY:

The individual in this role will be responsible for leading the TWIST and PROMISE programs aimed at enhancing community health and awareness.

KEY RESPONSIBILITIES:

  • Identify and enroll suitable participants for the TWIST and PROMISE programs.
  • Facilitate the implementation of the TWIST and PROMISE initiatives.
  • Work closely with the Program Director to recruit and engage an advisory group that convenes quarterly.
  • Assist clients in achieving their objectives by connecting them with community resources and making necessary referrals.
  • Engage in community outreach, networking, and advocacy within relevant social networks.
  • Adhere to TWIST and PROMISE protocols to enhance skills and self-efficacy related to protective health behaviors.
  • Provide education to clients regarding HIV risks, tailored to their personal circumstances.
  • Maintain accurate program data and prepare required reports.
  • Participate in regular meetings with the Project Coordinator and attend staff meetings as needed.
  • Ensure client confidentiality and comply with all relevant regulations and organizational policies regarding health information privacy.
  • Utilize online platforms for outreach to attract clients and share health messages.
  • Develop promotional materials for the programs.
  • Follow up with clients and referral organizations to monitor client engagement and progress.
  • Contact clients through various means to ensure program adherence and document interactions.
  • Organize and maintain client records and documentation.
  • Conduct assessments before and after interventions to evaluate clients' knowledge and attitudes towards HIV risk.
  • Engage in relevant training opportunities.
  • Perform additional duties as assigned.

This position requires adherence to all organizational standards, including conduct, policies, and compliance requirements, which will be part of regular performance evaluations.

QUALIFICATIONS:

  • Some post-secondary education is required; a Bachelor's degree in a health-related field is preferred.
  • Bilingual in English and Spanish is preferred.
  • A minimum of one year of experience working with diverse communities, particularly the LGBTQ+ of color community.
  • Understanding of HIV prevention, transmission, and harm reduction strategies.
  • Availability to work evenings, weekends, and in high-risk environments.
  • Experience in community outreach is preferred.

WORKING CONDITIONS: OSHA Category 2 – Normal work practices involve minimal exposure to blood or bodily fluids, though occasional exposure may occur.
