POSITION SUMMARY:
The PrEP Coordinator is responsible for supporting the Program Coordinator in executing program strategies. This role involves assisting participants in obtaining PrEP, ensuring adherence to medication, and facilitating retention in the program. The coordinator will work collaboratively to deliver HIV and HCV test results, conduct follow-ups, and engage in promotional activities for PrEP, including outreach initiatives.
The individual will lead efforts for those testing negative, providing education on safe practices and PrEP, coordinating necessary lab work, and addressing medication cost barriers to ensure adherence. The role will also focus on the development and enhancement of a low-threshold HIV Pre-Exposure Prophylaxis (PrEP) program.
KEY RESPONSIBILITIES:
- Conduct confidential assessments to identify the HIV prevention needs of participants, regardless of test outcomes.
- Establish and maintain resources for PrEP/PEP services in the community, fostering relationships with service providers.
- Facilitate access to biomedical prevention services, ensuring participants receive follow-up and support for adherence.
- Provide referrals for STI screenings and ensure participants have access to necessary services.
- Collaborate with project staff on advertising and media strategies for the HIV testing program.
- Administer HIV antibody testing both on-site and through community outreach.
- Deliver test results and offer partner services to individuals who test positive for HIV.
- Conduct follow-up assessments to reinforce participants' understanding of their risk levels.
- Work with Disease Intervention Specialists to connect participants to prevention services and screenings.
- Document program activities and report outcomes as required.
- Participate in ongoing training and development activities.
- Engage in community events and organizational meetings as needed.
- Perform additional duties as assigned.
This position requires adherence to all organizational standards, policies, and regulatory compliance requirements.
QUALIFICATIONS:
- Minimum of two years of experience in patient navigation, case management, or related fields.
- Preferred experience in HIV/AIDS services.
- Strong knowledge of community health resources and social service agencies.
- Experience working with diverse populations, including LGBTQ, African American, Latino, and low-income communities.
- Familiarity with HIV prevention interventions and STIs.
- Comfortable working in clinical settings with healthcare professionals.
SKILLS:
- Bilingual in English and Spanish preferred.
- Excellent communication and interpersonal skills.
- Ability to build relationships with individuals from various backgrounds.
- Proficient in computer applications and phone etiquette.
- Strong organizational skills and attention to detail.
EDUCATION:
- High School Diploma or GED required.
- Bachelor's degree in social work, public health, or a related field preferred.
WORKING CONDITIONS:
Exposure to blood and body fluids is not a condition of employment. The role may require occasional lifting of up to 20 pounds and extended periods of standing or sitting.
