SUMMARY OF POSITION:

The HIV Support Specialist is responsible for delivering both direct and indirect services aimed at facilitating follow-up, referrals, and data management for individuals affected by HIV, behavioral health challenges, and substance use issues. This role emphasizes client engagement and the establishment of robust relationships with individuals living with HIV. The Specialist will assist clients in maintaining their medical care and adhering to prescribed treatments, while actively working to eliminate barriers to healthcare access by identifying essential resources and guiding clients through healthcare systems.

The Specialist will particularly focus on participants aged 13-18 who identify as LGBTQ or are at high risk, providing follow-up services related to HIV testing and counseling, as well as behavioral health and substance misuse screenings. Responsibilities include administering assessments, entering data, and managing referrals as necessary. Upholding client confidentiality and adhering to HIPAA regulations is paramount, as is representing the organization with professionalism in community settings.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Offer advocacy, information, and resources to ensure clients are linked to appropriate care, acting as a liaison among various stakeholders.
  • Conduct training sessions for HIV testing and counseling.
  • Implement programs tailored for youth and facilitate necessary referrals.
  • Maintain accurate logs of participants in educational and outreach activities.
  • Engage with clients through various communication methods to assess needs and ensure their connection to HIV care and related services.
  • Participate in community events to support screening initiatives and provide referrals.
  • Manage data entry and maintain participant records in compliance with organizational standards.
  • Collaborate with a multidisciplinary team and interact with a diverse clientele.
  • Ensure all participant documentation is complete and meets contractual obligations.
  • Perform general administrative tasks, including data entry and scheduling.
  • Engage in ongoing professional development and training.
  • Attend organizational meetings and events to foster community engagement.
  • Execute additional responsibilities as assigned.

QUALIFICATIONS:

  • Experience working with LGBTQ/MSM Latino and African-American youth in underserved communities.
  • Understanding of HIV and available community resources is preferred.
  • Prior experience in social services or HIV-related fields is advantageous.
  • Certification as an HIV Test Counselor or similar training is preferred.

SKILLS:

  • Bilingual in English and Spanish is preferred.
  • Strong interpersonal and communication skills are essential.
  • Ability to build rapport with individuals from diverse backgrounds.
  • Demonstrated sensitivity and judgment in handling various client situations.
  • Proficiency in Microsoft Office applications.

LICENSURE:

  • Valid driver's license and access to a reliable vehicle.

EDUCATION:

  • High School Diploma or equivalent required; Bachelor's degree in a relevant field is preferred.

WORKING CONDITIONS:

This position may involve exposure to health-related environments, but such exposure is not a condition of employment.

Compensation: $40,000
