About the Role

We are seeking a highly motivated and detail-oriented Community Outreach Coordinator to join our team at Health Plan San Joaquin. As a key member of our outreach team, you will play a critical role in educating the community about our programs and services, establishing and maintaining relationships with community partners, and promoting our mission to improve the health of our community.

Key Responsibilities
  • Outreach and Education: Attend approved outreach events, distribute materials, and develop leads by conducting pre-screening of prospective members for enrollment into our programs.
  • Community Partnerships: Represent Health Plan San Joaquin at approved community meetings to network, identify potential partnerships, and promote our services.
  • Event Planning: Assist in event preparation, including setup and takedown.
  • Customer Engagement: Receive and initiate calls to potential members to provide information, conduct initial eligibility, and follow up on leads.
  • Documentation: Create required documentation, including call logs and other tracking systems.
Requirements
  • Education: High school diploma or general education degree, and one year of experience in a similar position, or equivalent combination of education and experience.
  • Skills: Basic knowledge of the eligibility requirements and benefits of Medi-Cal, ACA, and other state-sponsored programs; basic knowledge of community resources that assist the underserved; strong oral and written communication skills; and ability to work independently and as part of a team.
  • Preferred Qualifications: Associates degree and proficiency in one of Health Plan San Joaquin's threshold languages.
