Position Title: Community Support Specialist

Employment Type: Full-time

Role Summary:

As a Community Support Specialist, you will engage with various programs within our residential services, embodying the principles of person-centered care in your designated area. Your role will be pivotal in ensuring that individuals receive support tailored to their needs, fostering an environment of reassurance, stability, and collaboration among team members and those we serve. This position is vital during extended vacancies or when additional personnel are required.

Primary Responsibilities:
  • Support Functions:
    • Empower individuals to lead self-directed lives and engage fully in their communities.
    • Implement individual support plans, addressing needs and emergencies as they arise.
    • Guide individuals in understanding personal preferences, food preparation, hygiene, and health monitoring.
    • Administer medications in accordance with regulations, ensuring proper labeling and availability.
    • Facilitate transportation for recreational, employment, or medical appointments.
  • Communication and Team Collaboration:
    • Maintain transparent communication with supervisors and colleagues.
    • Encourage respectful and positive interactions with supported individuals.
    • Involve individuals in community outings and activities.
    • Contribute to agency operations, offering insights for enhancement.
  • Operational and Safety Responsibilities:
    • Adhere to safety protocols for the use of adaptive equipment.
    • Monitor and document health conditions, fulfilling daily reporting requirements.
    • Ensure a safe and sanitary work environment, managing corporate and individual funds responsibly.
    • Promptly report maintenance and health/safety concerns.
Expectations:
  • Possess a valid driver's license, complying with all driving regulations.
  • Exhibit a professional appearance and conduct, adhering to dress codes and standards.
  • Complete necessary certifications and stay informed about agency policies and procedures.
  • Be flexible with scheduling, including last-minute adjustments, committing to a minimum of 35 hours per week.
  • Participate in initiatives aimed at enhancing the agency's sustainability and reducing its environmental impact.
Physical Requirements:
  • Ability to lift up to 50 lbs and assist in positioning individuals.
  • Perform physical tasks such as walking, cleaning, and maintaining a professional demeanor.
  • Manage potential conflicts and tensions with a constructive attitude.
  • Execute trained restraints as necessary, in accordance with behavior support plans.
Qualifications:
  • High School Diploma or GED.
  • Strong communication skills, both written and verbal.
  • Basic proficiency in math, writing, and computer skills.
  • Demonstrated resourcefulness, proactivity, and organizational skills under pressure.
Travel Requirements:
  • Travel is a component of this position.
Additional Qualifications:
  • Must successfully pass a criminal background check.
  • Valid driver's license required.
  • Ability to independently evacuate premises in emergencies.
