About Mobile Healthcare Partners Inc: We are a dedicated home-based primary care organization focusing on geriatric health services, primarily catering to patients across New York State.

Role Summary: The Chronic Care Management (CCM) program provides tailored and proactive assistance to ensure patients and their healthcare providers maintain communication between scheduled visits. We are in search of a committed Enrollment Specialist to enhance our CCM initiative. In this role, you will be tasked with enrolling qualified patients into our CCM program, guaranteeing they receive essential care and support for effective management of their chronic health issues.

Key Responsibilities:

  • Receive lists of eligible patients without the need for cold outreach.
  • Engage with eligible patients to inform them about the advantages of our CCM program.
  • Guide patients through the enrollment process, ensuring clarity on all necessary information.
  • Work closely with the care management team to facilitate a smooth and efficient enrollment experience.
  • Possess a foundational understanding of Medicare coverage to assist patients with their payment inquiries.

Candidate Profile:

Experience: A minimum of 3-5 years in project-based healthcare roles.

Qualifications:

  • Education: Associate's Degree preferred; High School Diploma required.
  • Background in administrative support, enrollment processes, or customer service.
  • Experience in a medical office setting or familiarity with medical terminology and healthcare practices is preferred.
  • Proficient in Microsoft Office and Electronic Health Record Systems.
  • Knowledge of CCM programs and healthcare regulations is advantageous.
  • Strong attention to detail is essential.

Contractual Information:

This position includes one permanent full-time role and two contract roles, each lasting six months. These contract opportunities provide flexibility for those seeking temporary employment while maintaining the same responsibilities as full-time staff, playing a vital role in the success of our Chronic Care Management program.
