- Engage with clients, families, and caregivers to align with care plan objectives.
- Coordinate follow-up care post-hospitalization or emergency visits.
- Work collaboratively with healthcare team members and providers to ensure optimal health outcomes.
- Conduct outreach to patients to promote adherence to treatment plans.
- Maintain accurate records and databases of care coordination efforts.
- Current Licensed Practical Nurse (LPN) certification is mandatory.
- A minimum of 2 years of experience in a healthcare environment, with a preference for primary care experience.
