  • Proactively identify potential health crises and implement strategies to mitigate risks.
  • Assist patients in accessing healthcare services, social support, and community resources.
  • Engage with clients, families, and caregivers to align with care plan objectives.
  • Coordinate follow-up care post-hospitalization or emergency visits.
  • Work collaboratively with healthcare team members and providers to ensure optimal health outcomes.

Qualifications:
  • Current Licensed Practical Nurse (LPN) certification is mandatory.
