WHAT YOU'LL DO

The Care Coordinator-HARP manages a designated caseload of clients enrolled in the health home initiative and in a Health and Recovery Plan (HARP). Beyond providing essential health home care coordination services, the Care Coordinator-HARP aids clients in accessing Home and Community Based Services (HCBS) and Community Oriented Recovery and Empowerment Services (CORE). The role includes conducting the NYS Eligibility Assessment, developing and maintaining the HCBS Plan of Care, and connecting clients with HCBS and CORE providers in the community. Additionally, the Care Coordinator-HARP educates clients about HARP benefits and supports other care coordinators at JFS in linking their HARP-enrolled clients to necessary services.

Key Responsibilities
  • Manage a caseload of clients enrolled in HARP, ensuring compliance with health home policies and standards of care.
  • Conduct home visits and engage with clients in various community settings, including medical appointments and residential facilities.
  • Perform comprehensive assessments and create a client-centered Plan of Care tailored to individual needs.
  • Develop crisis plans for all clients and review them at least annually.
  • Facilitate annual case reviews with clients and their care teams to evaluate the Plan of Care and address any barriers to achieving goals.
  • Complete HARP Assessment training in the Uniform Assessment System for New York (UAS-NY) and perform assessments/reassessments while maintaining an active Health Commerce System (HCS) account.
  • Monitor HCBS service delivery to ensure clients are progressing towards their care plan goals.
  • Assist clients with appointment coordination, including scheduling and transportation arrangements.
  • Collaborate closely with the interdisciplinary care team, including primary care providers and mental health specialists.
  • Research community resources and government benefit programs to determine eligibility and provide appropriate referrals.
  • Utilize culturally sensitive and linguistically appropriate methods to engage clients.
  • Document all client interactions and engagement efforts accurately, submitting progress notes within 48 hours.
  • Engage in case conferences with the MCO HARP Care Manager and other providers to ensure ongoing client support.
  • Effectively use electronic systems, including Netsmart and HEALTHeLink.
  • Educate team members on HARP, HCBS, and CORE services and assist with client transitions as needed.
  • Participate in agency training and staff meetings as required.
  • Support health home programs as necessary.
WHAT YOU'LL BRING
  • A Bachelor's degree in fields such as child & family studies, community mental health, counseling, nursing, social work, or related areas, along with two years of experience providing direct services to individuals with serious mental illness or developmental disabilities.
OR
  • A Bachelor's degree in any field with three years of experience working directly with individuals with behavioral health diagnoses.
OR
  • A Bachelor's degree in any field and two years of experience as a Health Home care manager serving the SMI or SED population.
  • Demonstrated ability to work effectively in a culturally and linguistically diverse environment.
  • Strong skills in internet research and proficiency in software such as Outlook, Word, Excel, and web-based health information systems.
  • Bilingual skills are a plus; fluency in English is required.
  • Basic knowledge of chronic conditions, including serious mental illness and HIV/AIDS.
  • Willingness to work some evenings and weekends as needed.
This position requires travel between sites, necessitating a reliable vehicle, valid driver's license, and appropriate liability insurance. Additionally, proof of Covid-19 vaccination is required upon hiring.

  • Competitive salary range of $25.00-$30.00 per hour, based on experience and qualifications.
