KEY RESPONSIBILITIES
The HARP Care Coordinator manages a designated caseload of clients enrolled in the health home program and participating in a Health and Recovery Plan (HARP). In addition to delivering care coordination services, the Care Coordinator:
  • Facilitates access to Home and Community Based Services (HCBS) and Community Oriented Recovery and Empowerment Services (CORE).

The Care Coordinator conducts the NYS Eligibility Assessment, formulates, and maintains the HCBS Plan of Care, and connects clients with HCBS and CORE providers in the community. Additionally, the Care Coordinator:

  • Educates clients about HARP benefits and assists fellow care coordinators in linking their HARP-enrolled clients to available services.

Responsibilities Include:
- Maintaining a caseload of HARP clients and providing care coordination in line with health home policies and standards.
- Conducting home visits and engaging with clients in various community settings, including medical appointments and residential facilities.
- Performing comprehensive assessments and developing client-centered Plans of Care to meet individual needs.
- Assisting clients in achieving their goals as outlined in their Plans of Care.
- Completing crisis plans with each client and reviewing them at least annually.
- Conducting annual case reviews with clients and their care teams to evaluate Plans of Care and address any barriers to goal achievement.

Successfully completing HARP Assessment training in the Uniform Assessment System for New York (UAS-NY) is essential. Responsibilities also include:
- Monitoring HCBS service delivery to ensure clients are progressing towards their goals.
- Coordinating appointments, including scheduling and providing reminders.
- Collaborating closely with the interdisciplinary care team, including primary care providers and mental health specialists.

Researching community resources and government benefit programs to determine eligibility and provide appropriate referrals is also part of the role. Utilizing culturally sensitive and linguistically appropriate methods to engage clients is crucial. All interactions and efforts towards client engagement must be accurately documented, with progress notes submitted within 48 hours of client encounters.

Collaboration with the MCO HARP Care Manager, HCBS, and CORE providers is vital to ensure ongoing client engagement in services. Proficiency in electronic systems, including Netsmart and HEALTHeLink, is required.

WHAT YOU'LL BRING
A Bachelor's degree in fields such as child & family studies, community mental health, counseling, nursing, psychology, social work, or related areas, along with:
- Two years of experience providing direct services to individuals with serious mental illness or developmental disabilities, or
- A Bachelor's degree with three years of experience working directly with individuals with behavioral health diagnoses, or
- A Bachelor's degree and two years of experience as a Health Home care manager serving the SMI or SED population.

Demonstrated ability to work effectively in an inclusive and culturally diverse environment is essential. Strong computer skills, particularly with health information systems, are required. Bilingual skills are a plus, and fluency in English is mandatory. A basic understanding of chronic conditions, including serious mental illness, is necessary. This position involves travel, so a reliable vehicle and valid driver's license are required.

WHAT WE OFFER
Competitive salary range of $25.00-$30.00 per hour, based on experience and qualifications. Benefits include health insurance, accrued Paid Time Off (PTO), a 401k retirement plan with company match, and numerous observed holidays. We offer a reduced full-time work week of 35 hours and an early close on Fridays, along with a collaborative work environment that allows for meaningful impact within a forward-thinking organization.
