KEY RESPONSIBILITIES

  • Manage a caseload of clients enrolled in the Health and Recovery Plan (HARP), ensuring compliance with health home policies and standards.
  • Conduct home visits and engage with clients in various community settings, including medical appointments and residential facilities.
  • Perform comprehensive assessments and create individualized Plans of Care that address client needs and aspirations.
  • Develop crisis plans for all clients and conduct annual reviews to ensure ongoing support.
  • Complete HARP assessments and maintain an active account in the Health Commerce System.
  • Monitor the delivery of Home and Community Based Services (HCBS) to support clients in achieving their goals.
  • Assist clients with appointment coordination, including scheduling and transportation arrangements.
  • Collaborate with an interdisciplinary care team, including healthcare providers and community service organizations.
  • Research community resources and government programs to provide appropriate referrals and follow-up.
  • Utilize culturally sensitive approaches to engage clients effectively.
  • Document all client interactions and submit progress notes promptly.
  • Work closely with HARP Care Managers and service providers to ensure client engagement.
  • Leverage electronic systems for efficient case management.
  • Provide training to team members on HARP and HCBS services.
  • Participate in training sessions and staff meetings as required.
  • Support health home initiatives as needed.

QUALIFICATIONS

  • Bachelor's degree in fields such as social work, psychology, nursing, or related areas, along with two years of experience in direct service to individuals with serious mental illness or related conditions.
  • Alternatively, a Bachelor's degree in any field with three years of experience working directly with individuals with behavioral health issues.
  • Exhibit a strong ability to work in a diverse and inclusive environment.
  • Proficient in internet research and computer applications, including Microsoft Office and health information systems.
  • Bilingual abilities are a plus; fluency in English is required.
  • Demonstrate knowledge of chronic health conditions and mental health issues.
  • Willingness to work flexible hours, including some evenings and weekends.

This role requires travel between various locations, necessitating a reliable vehicle, valid driver's license, and appropriate insurance coverage. Additionally, proof of Covid-19 vaccination is required upon hiring.


WHAT WE PROVIDE

  • Competitive hourly wage ranging from $25.00 to $30.00, based on experience and qualifications.
