The Housing Support Coordinator will play a crucial role in assisting residents with essential connections to maintain a stable living environment of their choice.

- Support residents in establishing and maintaining utility services.
- Help with the acquisition of furniture and essential household items.
- Conduct monthly check-ins with residents to review their living situation and monitor their mental well-being.
- Document all interactions and visits in the resident's records.
- Practice person-centered approaches and exhibit a positive, compassionate demeanor when engaging with residents.
- Collaborate with residents and their service providers to ensure ongoing support.
- Assist residents in resolving housing-related disputes with landlords when necessary.
- Provide education, monitoring, and recovery-oriented support as needed for independent living.
- Work with residents to secure and maintain all necessary entitlements.
- Collaborate with residents and their support teams to develop personalized housing support plans.
- Provide transportation to residents when required.
- Ensure residents access required clinical services and other necessary linkages, including medical and vocational education.
- Assist new residents in applying for HUD / Section 8 housing.
- Maintain accurate records and reports as requested by the supervisor.
- Coordinate the moving process for residents as needed.
- Perform any additional duties necessary for the effective operation of the program.
A Bachelor's degree or four years of mental health experience or equivalent is required. One year of experience in the human services field is preferred; or an equivalent combination of education and experience.
*Additionally, this position requires a clean driving record as per company policy. The use of a personal vehicle is also necessary. Candidates must be at least eighteen (18) years of age.
Work EnvironmentThis position operates Monday through Friday from 8 am to 4:30 pm. There may be potential for hybrid work after a training period.
