The Housing Support Coordinator will play a crucial role in assisting residents with essential connections to maintain a stable living environment of their choice.

  1. Assist residents in finding safe and affordable housing options of their choice.

  2. Support residents in establishing and maintaining utility services.

  3. Help with the acquisition of furniture and essential household items.

  4. Conduct monthly check-ins with residents to review their living situation and monitor their mental well-being.

  5. Document all interactions and visits in the resident's records.

  6. Practice person-centered approaches and exhibit a positive, compassionate demeanor when engaging with residents.

  7. Collaborate with residents and their service providers to ensure ongoing support.

  8. Assist residents in resolving housing-related disputes with landlords when necessary.

  9. Provide education, monitoring, and recovery-oriented support as needed for independent living.

  10. Work with residents to secure and maintain all necessary entitlements.

  11. Collaborate with residents and their support teams to develop personalized housing support plans.

  12. Provide transportation to residents when required.

  13. Ensure residents access required clinical services and other necessary linkages, including medical and vocational education.

  14. Assist new residents in applying for HUD / Section 8 housing.

  15. Maintain accurate records and reports as requested by the supervisor.

  16. Coordinate the moving process for residents as needed.

  17. Perform any additional duties necessary for the effective operation of the program.
Qualifications

A Bachelor's degree or four years of mental health experience or equivalent is required. One year of experience in the human services field is preferred; or an equivalent combination of education and experience.

*Additionally, this position requires a clean driving record as per company policy. The use of a personal vehicle is also necessary. Candidates must be at least eighteen (18) years of age.

Work Environment

This position operates Monday through Friday from 8 am to 4:30 pm. There may be potential for hybrid work after a training period.
