Service Coordinator

Concern Housing is a non-profit agency dedicated to providing housing and support services to individuals and families. We are seeking a highly skilled Service Coordinator to join our team.
Key Responsibilities- Community Linkage: Facilitate residents' connection with community resources, including case management, clinical services, day programs, vocational services, and entitlements.
- Goal Planning and Progress Monitoring: Develop quarterly goal plans, functional assessments, and goal plan reviews. Conduct weekly meetings with clients to develop skills and review progress.
- Counseling and Support Services: Provide individual counseling and support services to residents.
- Housing Services: Assist with maintenance issues and provide daily living skills training, including medication monitoring and training, cleaning, and money management.
- Fire Safety Inspections: Perform monthly fire safety inspections to ensure resident safety.
- Compliance and Training: Remain current with all Agency required compliance documents and trainings.
We are seeking a candidate with a Bachelor's Degree in Psychology, Rehabilitation, Social Work, or a related mental health discipline, plus 2 years of experience working in a similar setting and/or working with people with mental illnesses. The ideal candidate will have excellent verbal and written communication skills, as well as the ability to make sound judgments regarding client care.
