Service Coordination Team Lead

**Job Summary**
Catholic Charities Family Community Services is seeking a highly skilled Service Coordination Team Lead to provide administrative oversight and support to the Program Supervisor in the Traumatic Brain Injury (TBI) Waiver and Nursing Home Transition and Diversion (NHTD) Waiver Service Coordination programs.
**Key Responsibilities**
- Provide support to the supervisor and staff, including hiring, training, and ongoing staff development.
- Complete administrative tasks as assigned, such as housing and UAS paperwork.
- Promote team building within the Care Coordination department and throughout the agency.
- Participate in and facilitate staff meetings within the Care Coordination Department.
- Review paperwork completed by staff to ensure high-quality work and regulatory compliance.
- Participate in quality management activities and provide direct service coordination to a limited caseload as needed.
- Monitor staff-assigned caseloads and provide data on capacity to the Program Supervisor or Department Director.
- Complete other paperwork required by CCFCS, DOH, and funders.
- Assist with client file reviews and ensure compliance with DOH regulations.
- Represent CCFCS on boards, committees, and task forces.
- Ensure compliance with local, state, and federal regulations and agency policies.
**Requirements**
- Bachelor's degree in human services or related field and one year of care management or case work with persons with disabilities.
- OR Associates degree in human services or related field and three years of care management or case work with persons with disabilities.
- Minimum of two years of supervisory experience preferred.
- Bilingual (English/Spanish) required for some positions.
- Ability to prioritize assignments, plan, and complete work projects with minimal direction.
- Excellent verbal and written skills.
- Demonstrate ethical business practices and ability to maintain confidentiality.
- Proficiency in Microsoft applications and ability to analyze and interpret data.
