POSITION OVERVIEW:

The mission of PATH is to empower individuals to lead more independent and fulfilling lives through effective teamwork.

This role primarily involves working with children and adolescents facing significant mental health challenges or emotional disturbances, along with their families, aiming to support youth in the least restrictive environments related to home, school, and community services.

Engagement with youth and their families is crucial in delivering the necessary support, training, and assistance for fostering a stable and healthy family and community life.


REQUIREMENTS:
A Bachelor's Degree in a Human Services discipline from an accredited institution.
One to two years of experience in the behavioral health sector.
Prior experience with children and adolescents is preferred.
Valid driver's license and personal vehicle usage are required.

KEY RESPONSIBILITIES:
Conduct comprehensive strengths-based assessments for each assigned child or youth.
Collaborate with each child, youth, and their families to create a cohesive service plan that addresses treatment needs, skill enhancement, social and emotional growth, and social service requirements.
Ensure adherence to the principles established by the Pennsylvania Child and Adolescent Service System Program (CASSP) in delivering services to youth and their families.
Coordinate psychiatric services as necessary.
