POSITION OVERVIEW:

The mission of PATH is to assist individuals in achieving greater independence and a fulfilling life through effective teamwork.

This role primarily involves working with children and adolescents facing significant mental health challenges or emotional disorders, along with their families, aiming to support the youth in the least restrictive environments concerning home, school, and community programs.

Collaboration with youth and their families is essential to provide the necessary support, training, and assistance for a stable and healthy family and community life.


REQUIREMENTS:
A Bachelor's Degree in a Human Services discipline from an accredited institution.
One to two years of experience in the behavioral health sector.
Preferred experience working with children and adolescents.
Possession of a valid driver's license and use of a personal vehicle is required.

KEY RESPONSIBILITIES:
Conduct comprehensive strengths-based assessments for each assigned child or youth.
Collaborate with each child/youth and their family to create a unified service plan that addresses treatment needs, skill development, social and emotional growth, and social service requirements.
Ensure adherence to the principles established by the Pennsylvania Child and Adolescent Service System Program (CASSP) in delivering services to youth and their families.
Coordinate psychiatric services as necessary.
