POSITION OVERVIEW:
The mission of PATH is to empower individuals towards achieving a more independent and fulfilling existence through effective teamwork.
This role primarily involves working with children and adolescents facing significant mental health challenges or emotional disturbances, alongside their families, with the objective of supporting youth in the least restrictive environments related to home, educational settings, and community services.
Engagement with youth and their families is crucial in delivering the necessary support, training, and assistance to foster a stable and healthy family and community life.
REQUIREMENTS:
A Bachelor's Degree in a Human Services discipline from an accredited institution.
One to two years of experience in the behavioral health sector.
Preferred experience with children and adolescents.
Possession of a valid driver's license and use of a personal vehicle is required.
KEY RESPONSIBILITIES:
Conduct a thorough strengths-based assessment for each assigned child or youth.
Collaborate with each child/youth and their family to create a cohesive service plan that addresses treatment needs, skill enhancement, emotional growth, and social service requirements.
Ensure adherence to the principles established by the Pennsylvania Child and Adolescent Service System Program (CASSP) in service delivery for youth and their families.
Facilitate coordination of psychiatric services as necessary.
