About the Position:

The Support Services Coordinator will play a key role in ensuring the smooth delivery of support services to our residents. This includes coordinating care plans, managing residence maintenance, and collaborating with colleagues to promote a positive and supportive team environment.

Duties and Responsibilities:

• Coordinate and implement individualized care plans in collaboration with residents, families, and healthcare professionals
• Conduct regular assessments to identify resident needs and develop targeted interventions
• Manage residence maintenance tasks, including cleaning, laundry, and meal preparation
• Ensure accurate and timely documentation of resident care and residence maintenance
• Participate in staff meetings and in-service training to enhance knowledge and skills
• Foster strong relationships with residents, colleagues, and community partners

Requirements:

  • High School diploma or equivalent required; experience in support services preferred
  • Ability to analyze complex situations and develop effective solutions
    li Excellent communication and interpersonal skills
  • Basic computer skills and familiarity with electronic health records
  • Valid Connecticut Driver's License and eligibility for auto insurance
Compensation and Benefits:

The salary range for this position is $45,000 - $55,000 per year, depending on experience. Network Human Services offers a comprehensive benefits package, including health insurance, paid time off, and opportunities for professional growth and development.
