We are seeking a compassionate and detail-oriented Care Coordinator Assistant to join our team at Network Human Services.
Salary InformationThe estimated salary for this role is $43,000 - $58,000 per year, based on experience.
Job SummaryThe Care Coordinator Assistant plays a vital role in providing high-quality support to residents at our community residence. As a member of our team, you will be responsible for assisting with daily living activities, implementing resident plans, and maintaining a safe and healthy environment.
Main Responsibilities- Direct Support: Provide direct care to residents, including assistance with activities of daily living, socialization, and development of meaningful relationships.
- Plan Implementation: Implement resident plans, including behavioral plans and habilitation skills, to ensure individualized care and support.
- Record Keeping: Maintain accurate electronic health records, including medication administration documentation and resident attendance.
- Maintenance and Safety: Participate in routine residence maintenance and ensure the safety and well-being of residents.
- Status Assessments: Document resident-specific status assessments, daily task analysis charts, and other relevant records.
- Education: High School diploma or GED.
- Certification: Medication certification required.
- Interpersonal Skills: Excellent interpersonal and communication skills.
- Confidentiality: Maturity and ability to handle confidential information.
- Proficiency: Proficiency in First Aid, CPR, PMT, Abuse/Neglect, and DDS Medication Administration.
- Computer Skills: Basic computer skills and experience with electronic health records preferred.
- Driver's License: Valid Connecticut Driver's License and eligibility for Network Human Services' auto insurance.
- Lifting Ability: Ability to lift, use adaptive equipment, and use physical restraint techniques as needed.
The work schedule for this role includes:
Thursday: 6:00am - 9:00am
Friday: 6:00am - 9:00am
Saturday: 7:00am - 10:00am
Sunday: 6:00am - 2:00pm
