Objectives: To oversee and enhance our Adult Family/Foster and Home Health Care services, ensuring the seamless operation of our programs.

Key Responsibilities:

  • Oversee the overall management of all Home Visits Registered Nurses;
  • Complete the MDS-HC and various clinical evaluations;
  • Continuously develop and assess each member's AFC care plan;
  • Train, evaluate, and supervise AFC caregivers in collaboration with the Program Director;
  • Review physician summaries;
  • Monitor the health conditions of all members to guarantee proper delivery of necessary AFC services;
  • Report any changes in a member's health status to their physician;
  • Periodically assess AFC caregiver logs;
  • Conduct admission visits for new members;
  • Document nursing progress notes for each visit or interaction;
  • Provide semiannual health-status reports to the member's primary care physician;
  • Plan and implement discharges from the AFC program;
  • Conduct orientation sessions for each AFC caregiver prior to their provision of personal care;
  • Offer ongoing training to AFC caregivers regarding health and aging.
  • Perform any additional office tasks as assigned by Program Directors and the President.

Required Skills:

  • Proven ability to manage staff while ensuring productivity, morale, and budget adherence.
  • Demonstrated capability to coach, train, and foster staff development.
  • Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint).

Education and Experience:

  • Master's or Bachelor's degree in Nursing.
  • Must possess a valid RN license from the Massachusetts Board of Registration.
  • A minimum of five years of prior experience in home health care.
