Nurse Manager for Adult Foster Care Services

Objectives: To oversee and enhance our Adult Family/Foster and Home Health Care services, ensuring the seamless operation of our programs.
Key Responsibilities:
- Oversee the overall management of all Home Visits Registered Nurses;
- Complete the MDS-HC and various clinical evaluations;
- Continuously develop and assess each member's AFC care plan;
- Train, evaluate, and supervise AFC caregivers in collaboration with the Program Director;
- Review physician summaries;
- Monitor the health conditions of all members to guarantee proper delivery of necessary AFC services;
- Report any changes in a member's health status to their physician;
- Periodically assess AFC caregiver logs;
- Conduct admission visits for new members;
- Document nursing progress notes for each visit or interaction;
- Provide semiannual health-status reports to the member's primary care physician;
- Plan and implement discharges from the AFC program;
- Conduct orientation sessions for each AFC caregiver prior to their provision of personal care;
- Offer ongoing training to AFC caregivers regarding health and aging.
- Perform any additional office tasks as assigned by Program Directors and the President.
Required Skills:
- Proven ability to manage staff while ensuring productivity, morale, and budget adherence.
- Demonstrated capability to coach, train, and foster staff development.
- Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint).
Education and Experience:
- Master's or Bachelor's degree in Nursing.
- Must possess a valid RN license from the Massachusetts Board of Registration.
- A minimum of five years of prior experience in home health care.
