Key Responsibilities:
  • Medication Administration: Administer medications to clients through various methods, ensuring compliance with service line protocols.
  • Nursing Evaluations: Conduct comprehensive nursing evaluations, including physical and mental health assessments, in accordance with service line standards.
  • Documentation: Accurately document medication administration and nursing evaluations, adhering to established service line protocols.
  • Medication Reviews: Perform medication administration record (MAR) reviews, monitor medication inventory, and prepare medications as necessary.
  • Client Communication: Engage with clients regarding medication inquiries and needs, facilitating refill requests to providers as per standard operating procedures.
  • Client Intake: Assist in client intake by verifying data and medical history, confirming the purpose of visits, and obtaining necessary preliminary health information.
  • Record Keeping: Maintain up-to-date and accurate medical and medication records, assisting with laboratory and phlebotomy tasks as required.
  • Resource Support: Serve as a resource for nursing and medical inquiries, overseeing health-related issues under the guidance of nursing leadership.
  • Meeting Participation: Attend all mandatory meetings as assigned.
  • Additional Duties: Carry out other related responsibilities as assigned by nursing leadership.

Qualifications:
  • Current licensure as a Practical Nurse (PN) in the relevant state.
  • Graduation from an accredited practical nursing program or an accredited healthcare assistant program.

Additional Skills:
  • Proficient in accurately entering, transcribing, and managing information in various formats.
  • Strong verbal and written communication skills, capable of producing clear reports and conveying complex ideas simply and respectfully.
  • Committed to confidentiality and the protection of sensitive information.
  • Competent in interpreting various documents, including laboratory results and health records.
  • Strong problem-solving skills and adaptability to new challenges.
  • Willingness to learn and collaborate with diverse teams.
