Fortcom Health Services is in search of a meticulous professional to fill the role of Supervisor for Adult Family/Foster and Home Health Care operations. In this pivotal position, you will be instrumental in maintaining the efficient functioning of our services.

Key Responsibilities:
  • Overseeing Home Visit Registered Nurses
  • Conducting clinical evaluations
  • Creating and assessing AFC care plans
  • Training and managing AFC support staff
  • Monitoring the health conditions of members
  • Collaborating with medical practitioners
  • Performing on-site assessments
  • Preparing health-status documentation
  • Coordinating discharge planning and execution
  • Facilitating caregiver orientation and training sessions
  • Executing additional assigned administrative tasks
Required Skills:
  • Leadership and productivity management capabilities
  • Coaching and staff development expertise
  • Proficiency in Microsoft Office Suite
Qualifications:
  • Master's or Bachelor's degree in Nursing
  • Current RN licensure
  • At least five years of experience in home health care
