Key Responsibilities:- Overseeing Home Visit Registered Nurses
- Conducting clinical evaluations
- Creating and assessing AFC care plans
- Training and managing AFC support staff
- Monitoring the health conditions of members
- Collaborating with medical practitioners
- Performing on-site assessments
- Preparing health-status documentation
- Coordinating discharge planning and execution
- Facilitating caregiver orientation and training sessions
- Executing additional assigned administrative tasks
- Leadership and productivity management capabilities
- Coaching and staff development expertise
- Proficiency in Microsoft Office Suite
- Master's or Bachelor's degree in Nursing
- Current RN licensure
- At least five years of experience in home health care
