Job Summary

We are seeking a highly skilled and experienced Clinical Coordinator to join our team at Services For The Underserved, Inc. The successful candidate will be responsible for coordinating client services and case conferences, as well as providing direct supervision to case managers, housing specialists, and vocational specialists.

Key Responsibilities
  • Interview residents using standard case work techniques to determine suitability for residential services and assess their level of psychological functioning.
  • Conduct initial assessments and assign residents to a case manager, determining the level of care required.
  • Makes counseling-related referrals and serves as a liaison to mental health agencies as needed.
  • Develops and maintains linkages with service providers to ensure seamless care coordination.
  • Represents the program at community task force and conference meetings, promoting collaboration and partnerships.
  • Prepares client referral reports to supportive service providers and monitors and evaluates client progress records.
  • Provides clinical supervision and audits charts to ensure that notes are completed in a timely fashion by clinical staff.
  • Supervises the program in the Director's absence, ensuring continuity of care.
  • Attends all scheduled meetings and training sessions to stay up-to-date on best practices and industry developments.
Requirements
  • Master's Degree from an accredited institution with a major in social work or a related field.
  • LMSW or LMHC preferred.
  • Strong working knowledge of psychodynamics, group work, case work, health systems, and human and child development.
  • Experience working with domestic violence and an understanding of Post-Traumatic Stress Disorder and trauma-informed care is preferred.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to read and analyze documents for analytical purposes.
  • Working knowledge of AWARDS, CARES, and SORTS systems.
  • English and Spanish speaking is a plus.
