Position Summary

The Care Coordinator (LPN) is responsible for executing case management activities that cater to the needs of clients assigned to Care Lync Georgia. This role involves conducting initial assessments to determine the Level of Care, evaluating the suitability of Home and Community-Based Services (HCBS) for prospective clients, formulating individualized care management plans, implementing these plans, reassessing client needs, and monitoring clinical outcomes to ensure services are delivered in a professional, comprehensive, and cost-effective manner.

Key Responsibilities

  • Conduct thorough assessments to verify or refute nursing home level of care.
  • Perform medical and social evaluations under the guidance of a registered nurse.
  • Complete assessments in accordance with established guidelines and submit for review.
  • Demonstrate experience in case management within the healthcare sector.
  • Maintain an active nursing license in the state of Georgia.

Assessment Responsibilities

  • Arrange face-to-face consultations with clients.
  • Gather medical documentation necessary for assessments.
  • Create Comprehensive Care Plans tailored to individual client needs.

Submit completed assessments to the registered nurse supervisor for evaluation and recommendations.

Administrative Responsibilities

  • Assist in compiling statistical data.
  • Collaborate with the Management Team to schedule client appointments.
  • Participate in organizational meetings and training sessions.
  • Process bi-weekly invoices for completed assessments.

Work Environment

Employees typically work in an office or residential setting with moderate background noise.

Physical Demands

  • Employees may need to stand, walk, sit, reach, handle objects, climb, balance, stoop, kneel, crouch, or crawl.
  • Must possess the ability to read, write, and communicate effectively.
  • Regularly required to lift and carry up to 50 pounds.
  • Must be capable of pushing/pulling up to 25 pounds.
