Position Summary

The Care Coordinator (LPN) is responsible for essential case management activities tailored to meet the needs of clients assigned to Care Lync Georgia. This role includes conducting initial assessments to determine the appropriate Level of Care, evaluating the suitability of Home and Community-Based Services (HCBS) for prospective clients, creating personalized care management plans, executing the care plan, reassessing client needs, and monitoring clinical outcomes to ensure that services are delivered professionally, comprehensively, and cost-effectively.

Key Responsibilities

  • Conduct comprehensive assessments to verify or refute nursing home level of care.
  • Perform medical and social evaluations under the guidance of a registered nurse.
  • Complete assessments in accordance with established guidelines and submit for review.
  • Demonstrate experience in case management within the healthcare sector.
  • Maintain an active nursing license in the state of Georgia.

Assessment Responsibilities

  • Schedule in-person interviews with clients.
  • Gather medical documentation necessary for assessments.
  • Develop Comprehensive Care Plans tailored to individual client needs.

Submit completed assessments to the registered nurse supervisor for evaluation and recommendations.

Administrative Responsibilities

  • Assist in compiling statistical data.
  • Consult with the Management Team regarding client scheduling.
  • Participate in organizational meetings and training sessions.
  • Submit bi-weekly invoices for completed assessments.

Work Environment

Employees typically work in an office or residential setting with ambient background noise.

Physical Demands

  • Employees may need to stand, walk, sit, reach, handle objects, climb, balance, stoop, kneel, crouch, or crawl.
  • Must possess the ability to read, write, and communicate effectively.
  • Regularly required to lift and carry items weighing up to 50 pounds.
  • Must be capable of pushing or pulling weights up to 25 pounds.
