About the Role

We are seeking a highly skilled and compassionate Care Coordinator to join our team at View Point Health. As a Care Coordinator, you will play a critical role in providing wraparound services to youth and families across the state of Georgia.

Key Responsibilities
  • Develop and implement individualized plans of care for youth and families, ensuring a family-centered and strength-based approach.
  • Convene and facilitate monthly Child and Family Team (CFTM) meetings, ensuring active participation from family members, therapists, doctors, and community resources.
  • Coordinate the delivery of services, including educational, social, and other support services, to meet the needs of youth and families.
  • Develop and implement crisis and safety plans, ensuring the well-being and safety of youth and families.
  • Monitor and track progress, ensuring that youth and families meet their goals and achieve self-sufficiency.
Requirements
  • High School diploma or equivalent, with three years of experience in a social services-related position.
  • Preferred qualifications include a Bachelor's degree in social services or a closely related field, with two or more years of experience in care coordination and working with adults with severe and persistent mental illness and addictive diseases.
  • Superior verbal and written communication skills, with the ability to multi-task and manage time effectively.
  • Knowledge of community resources within the Savannah area, with a valid Georgia driver's license and a clean Motor Vehicle Record.
