About the Role:

We are seeking a Case Manager I to provide coordination and support to clients/family in their transition from nursing facilities/long term residential facilities with the goal of coming back into their community. The ideal candidate will have a strong understanding of the needs of individuals with developmental disabilities and be able to provide effective support and guidance to help them achieve their goals.

Key Responsibilities:

  • Help clients obtain housing readiness documents, such as ID, Social Security Card, birth certificate, and income verification
  • Research new housing opportunities and apply clients to all housing opportunities available to them
  • Maintain good relationships with landlords/housing agencies/housing authority
  • Help clients attend housing interviews/meetings
  • Assist clients to schedule and travel to all medical and dental appointments
  • Money Management (e.g. banking and budgeting)
  • Ensure that your client has access to free or low-cost transportation in the community
  • Connect your client to community programs and benefits
  • Act as liaison between EBI and other State and Local agencies, i.e. Housing Authority and Social Security
  • Maintain accurate and detailed records, including State and program required documentation, case notes, billing information, and client file
  • Create a comprehensive service plan to ensure ongoing services in the community and support in transitioning of institutional settings to housing

Requirements:

  • Two years of experience working in the field of human services; experience working with programs such as IHSS and Medi-Cal preferred
  • Bachelor's degree (education can be substituted with additional 2 years of experience in the field)
  • Possession of valid California driver's license with a driving record that meets agency standards. Auto insurance and access to a vehicle
