About the Role

East Bay Innovations is seeking a skilled Case Manager I to join our Transitional Care and Case Management department. As a Case Manager I, you will play a vital role in supporting clients and their families in transitioning from nursing facilities or long-term residential facilities to community-based settings.

Key Responsibilities:

  • Coordinate and support clients in securing affordable housing and navigating government benefits.
  • Help clients locate adequate services, such as In-Home Support Services (IHSS) or nursing support.
  • Maintain accurate and detailed records, including state and program required documentation, case notes, billing information, and client files.
  • Create comprehensive service plans to ensure ongoing services in the community and support in transitioning from institutional settings to housing.

Requirements:

  • Two years of experience working in the field of human services, with experience working with programs such as IHSS and Medi-Cal preferred.
  • Bachelor's degree (education can be substituted with additional 2 years of experience in the field).
  • Possession of a valid California driver's license with a driving record that meets agency standards. Auto insurance and access to a vehicle.
