Case Manager

At the Regional Center of the East Bay, we are seeking a highly motivated and compassionate Case Manager to join our team. As a Case Manager, you will play a vital role in supporting individuals with developmental disabilities and their families in achieving their goals and improving their quality of life.
Key Responsibilities:- Develop and implement individualized plans to meet the unique needs of each consumer, including assessment, evaluation, planning, and monitoring.
- Coordinate and support the implementation of services, including identifying natural supports and generic services, and evaluating when a need exists that can only be met through RCEB authorized funding.
- Advocate on behalf of consumers to protect their rights under the law and ensure they receive those services and benefits to which they are entitled.
- Maintain accurate and up-to-date documentation and records, including consumer files, service plans, and progress reports.
- Collaborate with other professionals, including healthcare providers, educators, and community agencies, to ensure comprehensive support for consumers.
- Provide education and support to consumers and their families on available services, benefits, and resources.
- Bachelor's degree in Social Work, Psychology, or a related field, or equivalent experience.
- At least 2 years of experience working with individuals with developmental disabilities, preferably in a case management or related role.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work effectively in a fast-paced environment and prioritize multiple tasks and responsibilities.
- Valid driver's license and reliable transportation.
