Job Title: Case Manager I/II

HealthRIGHT 360 is seeking a highly skilled Case Manager I/II to join our team. As a Case Manager, you will work with a multidisciplinary team to provide casework services to clients, handle intake and initial placement in community-based services, transportation, case plan, case management, and interface with Division of Adult Parole Operations (DAPO) to assist clients in navigating the needed services to reach health and recovery.

Responsibilities:
  • Maintains an assigned caseload of up to fifty (50) CDCR clients.
  • Provides learning experience opportunities and offers clinical support to assist clients in meeting their treatment goals.
  • Develops Case Management Plans (CMP) for clients.
  • Develops and monitors clients' progress relative to their CMP through all of phases of the program and links them to both internal and external resources based on their treatment needs.
  • Maintains progress notes in clients' files.
  • Keeps AOR apprised of clients' progress and develops exit and aftercare plans.
  • Travels to CBPs, sometimes requiring overnight stays, to provide outreach, case management, and assessment services.
  • Corresponds with AOR and assigns authorization for readmissions and client transfers.
Requirements:
  • High school diploma or equivalent.
  • Minimum 2 years' case management experience with criminal justice population.
  • First Aid and CPR certified within 30 days of employment.
  • Possess a valid CA driver's license.
  • Familiarity with community corrections systems and current community corrections practices.
  • Familiarity with community-based programs.
  • Ability to enter data into various electronic systems while maintaining the integrity and accuracy of the data.
  • Computer literacy and proficiency in Microsoft Office.
  • Integrity to handle sensitive information in a confidential manner.
  • Ability to work cooperatively with a multidisciplinary team and to work with clients of diverse ethnic and cultural backgrounds.
  • Ability to work within a frequently changing project scope while maintaining overall direction and structured priorities.
  • Bilingual preferred.
