Job Title: Case Manager

At the Regional Center of the East Bay, we are seeking a skilled and compassionate Case Manager to join our team. As a Case Manager, you will play a vital role in supporting individuals with developmental disabilities and their families in achieving their goals and living fulfilling lives.

Key Responsibilities:
  • Develop and implement individualized plans to meet the unique needs of each consumer, ensuring their rights and interests are protected.
  • Collaborate with consumers, families, and community resources to provide comprehensive support and services.
  • Conduct assessments, evaluations, and monitoring to ensure the effectiveness of services and make adjustments as needed.
  • Advocate on behalf of consumers to ensure they receive the services and benefits they are entitled to.
  • Maintain accurate and detailed records of consumer interactions, services provided, and outcomes.
Requirements:
  • Bachelor's degree in Social Work, Psychology, or a related field, or equivalent experience.
  • 2 years of experience working with individuals with developmental disabilities, preferably in a case management or social services role.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to speak Cambodian is preferred but not required.
