Job Title: Case Manager

At the Regional Center of the East Bay, we are seeking a skilled Case Manager to join our team. As a Case Manager, you will play a vital role in supporting individuals with developmental disabilities to achieve their goals and live fulfilling lives.

Key Responsibilities:
  • Develop and implement individualized plans to support consumers in achieving their goals and improving their quality of life.
  • Coordinate and facilitate the planning process, including assessment, evaluation, and monitoring of services.
  • Advocate on behalf of consumers to protect their rights and ensure they receive the services and benefits they are entitled to.
  • Maintain accurate and up-to-date documentation of consumer interactions and services provided.
  • Collaborate with other professionals, including healthcare providers, educators, and community agencies, to ensure comprehensive support for consumers.
Requirements:
  • Bachelor's degree in Social Work, Psychology, or a related field, or equivalent experience.
  • Ability to communicate effectively, both verbally and in writing.
  • Valid driver's license and reliable transportation.
  • Cultural awareness and sensitivity, with the ability to work effectively with diverse populations.
  • Ability to speak and read in Mandarin and/or Cantonese (for bilingual positions).
