Case Manager

At the Regional Center of the East Bay, we are seeking a skilled Case Manager to join our team. As a Case Manager, you will play a vital role in supporting individuals with developmental disabilities in achieving their goals and living fulfilling lives.
Key Responsibilities:- Develop and implement individualized plans to support consumers in achieving their goals and improving their quality of life.
- Coordinate and support the implementation of services and supports to meet the unique needs of each consumer.
- Advocate on behalf of consumers to protect their rights and ensure they receive the services and benefits they are entitled to.
- Maintain accurate and up-to-date documentation of consumer interactions and services provided.
- Collaborate with other professionals and community resources to ensure comprehensive support for consumers.
- Bachelor's degree in Social Work, Psychology, or a related human services field, or equivalent experience.
- Ability to communicate effectively with consumers, families, and other stakeholders.
- Valid driver's license and reliable transportation.
- Cultural awareness and sensitivity, with the ability to work effectively with diverse populations.
