Position Summary: Are you dedicated to enhancing the welfare of children? The role of Residential Program Case Coordinator is designed for individuals who aspire to positively impact the lives of children and families. In this capacity, you will liaise with local and state agencies to facilitate clinical and case management services aimed at achieving the long-term objectives of children and their families.

Program Overview: The Lee Randall Jones Community Residence, accredited by the Office of Mental Health, operates as an eight-bed facility in Cheektowaga. This residence caters to children aged 5-14 who face primary psychiatric challenges that do not necessitate acute care but require structured support from a community-based program. The facility offers round-the-clock supervision along with access to medical, recreational, and psychiatric services that may extend post-discharge.

Daily Responsibilities of the Residential Program Case Coordinator:

  • Oversee clinical and case management for assigned children and families.
  • Collaborate with local and state agencies to ensure comprehensive care.
  • Gather and integrate information from diverse sources to conduct assessments, identify service requirements, and formulate treatment plans.
  • Employ various therapeutic interventions, including Trauma-Focused Cognitive Behavioral Therapy and Family Therapy.
  • Participate in Service Plan Reviews and ensure adherence to recommendations.
  • Maintain accurate case records and documentation in compliance with state regulations.
  • Engage families actively in the treatment process.
  • Initiate discharge planning upon admission and pursue this goal diligently.
  • Provide clinical leadership to the treatment team by advocating for trauma-informed care, collaborative problem-solving, and therapeutic crisis intervention strategies.
  • Contribute to the program's Quality Improvement initiatives.
  • Foster positive relationships with program staff and other stakeholders.
  • Adhere to all agency policies and procedures.

Qualifications:

  • A Bachelor's Degree in Social Work or a related discipline, with experience in child welfare; a Master's Degree is preferred.
  • A valid NYS Driver's License.
  • Experience in delivering trauma-informed care to emotionally disturbed children is preferred.
  • Ability to de-escalate and manage challenging behaviors exhibited by children.
  • Capability to engage in physical interventions when necessary.
  • Physical stamina to stand, walk, climb, stoop, kneel, and occasionally lift or move over 25 pounds.

Compensation: Competitive pay rate ranging from $25 to $27 per hour, commensurate with experience.
