Position Overview

We are seeking a Part-Time Housing Stability Case Manager/Data Manager for our Rapid Rehousing Program.

Our part-time roles offer:

POSITION OBJECTIVE:

The Housing Stability Case Manager/Data Manager will play a crucial role in assisting clients to swiftly transition out of homelessness and sustain stable housing. The service delivery model is rooted in a Housing First philosophy, developing tailored, person-centered housing stability support plans for each household. This outreach role involves working with families at various stages of housing relocation, including data entry and intake services to monitor and document all facets of the Rapid Rehousing Program.

Key Responsibilities

PRIMARY DUTIES AND RESPONSIBILITIES:

  • Identify and research affordable housing options for individuals facing homelessness, while engaging landlords to create housing opportunities for participants.
  • Educate landlords on the benefits of the Rapid Rehousing Program and address any potential barriers to their involvement.
  • Maintain a comprehensive understanding of the rental market, including Housing Quality Standards, Fair Market Rents, and Rent Reasonableness Standards.
  • Conduct housing needs assessments and evaluate eligibility for the Rapid Rehousing Program.
  • Provide direct case management or collaborate with case management to ensure continuity of services for participants.
  • Effectively communicate with local community organizations and external agencies to foster relationships that facilitate access to necessary resources.
  • Assist participants in locating suitable rental housing that aligns with their needs, preferences, and financial capabilities.
  • Support participants in negotiating reasonable lease agreements with landlords and developing skills to be successful tenants.
  • Maintain organized and up-to-date lease and compliance files, as well as all client case records.
  • Engage in ongoing outreach and creative engagement strategies with tenants.
  • Conduct thorough assessments and assist tenants in formulating action plans to achieve their goals.
  • Collaborate with tenants and property management to coordinate eviction prevention strategies and establish housing permanency plans.
  • Assist tenants in acquiring essential life skills, including understanding tenant rights and responsibilities, to maintain housing stability.
  • Connect tenants with employment opportunities, skill development resources, and access to medical, mental health, substance use, and psychosocial support services as needed.
  • Provide crisis intervention as necessary.
  • Adhere to documentation requirements as dictated by program needs and regulatory mandates.
  • Possess a working knowledge of Service Point and Homeless Management Information System (HMIS).
  • Collaborate closely with the Director of Residential Services and other case managers to coordinate services for families in emergency shelter.
  • Maintain ongoing communication with clients to ensure services are effectively supporting their stability.
  • Uphold confidentiality, respect client privacy, and preserve clients' routines and independence as much as possible.
  • Recognize the spiritual needs of individuals and refer them to appropriate pastoral care as necessary.
  • Assist the Development Department Grant Writer in identifying grant opportunities and contribute to fundraising efforts for the Rapid Rehousing Program.
  • Input all relevant data into the Homeless Management Information System and Service Point.
  • Prepare for inspections of program files and materials to ensure compliance with all requirements.
  • Generate reports as required by oversight staff and agencies.
  • Update and create new client forms as needed.
  • Represent the organization on committees and work groups as appropriate.
  • Participate in ongoing training and informational meetings related to homelessness services.
  • Attend all staff meetings, workshops, and training sessions as directed.
  • Engage in other duties and activities as assigned.
  • Complete all required training sessions.
