Child Welfare Case Coordinator

The Child Welfare Case Coordinator is responsible for delivering case management services to a designated group of children and families. This role involves assisting parents in fulfilling court-mandated obligations to promote family reunification and facilitating concurrent planning to ensure timely permanence for children when reunification is not feasible.
Key Responsibilities:
- Oversees a designated caseload of children within the Foster Care and Adoption program, providing essential casework services to ensure the safety and well-being of each child while supporting their permanency planning objectives.
- Conducts regular home visits with children in foster care, ensuring monthly contact at a minimum, with increased frequency based on individual needs.
- Maintains monthly communication with all identified parents of children in foster care.
- Organizes and supervises visitations between parents and their children in foster care, as well as with siblings placed in different settings.
- Offers casework support to biological parents to help them meet court requirements for family reunification; promotes concurrent planning to secure timely permanence for children when necessary.
- Collaborates with local district case managers and other service providers to facilitate the child's permanency plan through ongoing communication.
- Manages transitions for children moving between different care settings, such as from their biological parents' home to a foster home or vice versa.
- Participates in Family Court proceedings to advocate for the child's permanency plan.
- Supports the development of foster parents' skills to effectively meet the needs of the children in their care.
- Keeps accurate progress notes, case documentation, and required reports in line with district protocols and state regulations.
- Exhibits sensitivity, fairness, and acceptance of diversity in all interactions.
- Engages in program Quality Improvement and Utilization review processes.
- Fosters positive working relationships with program staff and other agency personnel.
- Demonstrates strong interpersonal skills in interactions with both internal and external stakeholders.
- Promotes the program's services as needed.
- Participates in the department's on-call rotation.
- Undertakes additional responsibilities as assigned by the Foster Care and Adoption Supervisor or Program Manager.
Qualifications:
- Bachelor's Degree in Social Work (BSW) with two years of relevant experience in child welfare preferred, or a Master's Degree in Social Work (MSW) with child welfare experience.
- Possession of a valid driver's license and ongoing automobile insurance.
- Access to reliable transportation during work hours.
- Ability to work collaboratively as part of a team.
Work Schedule: Primarily traditional business hours, with occasional evening and weekend hours required.
Compensation: Competitive hourly rate for a full-time work week.
