Position Overview

POSITION SCOPE AND OBJECTIVES:

The Housing Stability Case Manager/Data Management Specialist will play a crucial role in assisting clients in swiftly transitioning out of homelessness and achieving sustainable housing. Service delivery is rooted in a Housing First philosophy, crafting personalized, client-centered housing stability support plans for each household. This outreach role involves working with families at various stages of housing relocation, encompassing data entry and intake services that track and document all facets of the Rapid Rehousing Program at Buffalo Area Services.

Key Responsibilities

PRIMARY DUTIES AND RESPONSIBILITIES:

Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions.

  • Identify and research suitable affordable housing solutions for individuals facing homelessness, while engaging landlords to create housing opportunities for participants.
  • Educate landlords about the benefits of the Rapid Rehousing program and address any potential concerns regarding their participation.
  • Maintain a comprehensive understanding of the rental market, including Housing Quality Standards, Fair Market Rents, and Rent Reasonableness Standards.
  • Conduct housing needs assessments and determine participant eligibility for the Rapid Rehousing program.
  • Provide direct case management or collaborate with case management to ensure continuity of services for participants.
  • Effectively communicate with local community members and external agencies, fostering relationships that facilitate access to necessary resources.
  • Assist participants in locating appropriate rental housing that aligns with their needs, preferences, and financial capabilities.
  • Support participants in negotiating fair and manageable lease agreements with landlords while equipping them with skills for successful tenancy.
  • Maintain organized and up-to-date lease and compliance files, as well as all client case records.
  • Engage in ongoing outreach and creative engagement with tenants.
  • Conduct thorough assessments and assist tenants in formulating action plans to meet their goals.
  • Collaborate with tenants and property management to implement eviction prevention strategies and develop plans for housing permanence.
  • Help tenants cultivate essential life skills, including understanding tenant rights and responsibilities, to support housing stability.
  • Connect tenants with employment opportunities, skill development programs, and access to medical, mental health, substance use, and psychosocial support services as required.
  • Provide crisis intervention when necessary.
  • Adhere to documentation requirements as dictated by program needs and HUD regulations.
  • Possess a working knowledge of Service Point and the Homeless Management Information System (HMIS).
  • Collaborate closely with the Director of Residential Services, Residential Services Case Managers, and Pathway of Hope Case Manager to coordinate services for families in emergency shelter.
  • Maintain contact with clients to ensure that services continue to support their stability throughout the commitment period.
  • Uphold confidentiality, respect privacy, and preserve clients' routines and independence as much as possible.
  • Recognize the spiritual needs of individuals and refer them to appropriate pastoral care as necessary.
  • Assist the Development Department Grant Writer in identifying grant and RFP opportunities and support efforts to secure additional funding for the Rapid Rehousing Program.
  • Input all relevant data into the Homeless Management Information System and Service Point.
  • Prepare for inspections of program files and materials, ensuring compliance with all requirements.
  • Generate reports as needed for staff and oversight agencies.
  • Update and create new client forms as necessary.
  • Represent the organization on committees, work groups, and commissions as appropriate.
  • Participate in ongoing training and informational meetings in collaboration with local homeless alliances.
  • Attend all staff meetings, workshops, and training sessions as directed.
  • Engage in additional duties and activities as assigned.
  • Complete all required KeepSAfe training sessions.
