Role Summary

The Case Manager is responsible for managing the service planning and transition processes for youth entering and exiting our programs. This role is crucial in fostering a safe and effective learning environment that promotes both social and emotional growth, enabling youth to achieve their academic and personal aspirations.

Key Responsibilities

  • Actively participate in the Multidisciplinary Treatment Team (MDT).
  • Conduct intake assessments for youth upon entry into the program and perform re-assessments as per contractual obligations.
  • Develop, document, and evaluate individualized service/care plans based on the needs assessment, ensuring timely updates.
  • Facilitate academic, vocational, and behavioral assessments promptly upon enrollment, utilizing this data to inform the individual service/care plan.
  • Ensure the delivery of appropriate program services that cater to the youth's needs, aligning with the Personal Growth Model to enhance their interpersonal relationship skills.
  • Represent the program at court hearings, meetings, and legal proceedings involving youth.
  • Implement Evidence-Based Treatment models as per program requirements, maintaining fidelity to these models.
  • Organize and lead youth progress meetings with MDT members on a monthly basis or as necessary.
  • Coordinate transitional meetings 60 days before a youth's release and assist in crafting a Transition Plan 30 days prior to their program termination.
  • Ensure each youth undergoes a post-needs assessment and has a finalized transitional plan post-release.
  • Complete administrative responsibilities related to the role, including:
    • Providing comprehensive monthly reports to guardians, judges, and probation officers within required timelines.
    • Overseeing weekly behavioral updates and goal progressions by advisors.
    • Conducting monthly audits to verify the accuracy of recorded information.
    • Maintaining case management and service documentation.
    • Conducting and preserving youth exit interview data.
    • Arranging off-site appointments and transportation for youth.
  • Conduct home visits with youth's families as per program and state guidelines, aiding families in understanding the impacts of group living and family separation.
  • May facilitate or assist in programs such as ART, CYT, Girls Circle, The Council for Boys and Young Men, and Life Skills.
  • Operate program vehicles in accordance with transportation policies.
  • Attend mandatory training and professional development sessions, maintaining necessary certifications and licenses.
  • Assist with special projects and perform additional duties as assigned.

Qualifications

  • Bachelor's degree in Psychology, Social Work, or a related field; a Master's degree is preferred.
  • At least one year of experience working with seriously emotionally disturbed youth or those with substance abuse issues.
  • Ability to obtain and maintain crisis intervention and physical restraint training and certification as mandated by state regulations.
  • May need to secure current CPR and First Aid Certification from a nationally recognized organization.
