Mental Health Community Outreach Worker

Join our dynamic team at Pathways Vermont as a Mental Health Community Outreach Worker. We are seeking a skilled professional to work with us in providing innovative mental health services through ongoing supportive social work.

About the Job:

This role involves working with individuals who face mental health challenges, histories of homelessness, and substance use issues. As a member of our interdisciplinary team, you will provide flexible, compassionate, community-based services to promote stability, recovery, and community reintegration.

Key Responsibilities:
  • Build strong relationships with clients and deliver services using a client-centered approach.
  • Help clients access community resources and services, such as entitlements, benefits, advocacy, and addiction services.
  • Offer one-on-one support and guidance on life skills, housing concerns, and health maintenance.
  • Record and report client progress accurately and maintain confidentiality.
  • Be part of our 24/7 on-call services rotation with other team members.
Requirements and Qualifications:
  • Demonstrated expertise in interpersonal and counseling skills, particularly when working with marginalized groups.
  • Effective communication, writing, and computer skills.
  • Ability to think independently and handle challenging situations.
  • Previous experience in mental health or harm reduction settings is preferred.
  • A valid driver's license and reliable transportation.
