Job SummaryWe are seeking a highly skilled Care Manager to join our Community First Choice (CFC) Program team. As a Care Manager, you will play a pivotal role in coordinating and managing personalized care services for our clients, ensuring they receive the support they need to live independently and comfortably in their communities.
Key Responsibilities- Conduct thorough assessments to evaluate the needs and preferences of our participants.
- Develop and implement tailored care plans in collaboration with participants, their families, and other caregivers.
- Oversee the provision of services, ensuring alignment with the goals and needs of our clients.
- Provide ongoing support and advocacy for our participants, proactively addressing concerns and needs.
- Maintain accurate and up-to-date records of all services and interactions.
- Collaborate with community resources and service providers to strengthen the support network for our clients.
- Engage in ongoing professional development opportunities to stay current with best practices in care management.
- Bachelor's degree in Social Work, Nursing, or a related field.
- Minimum of 2 years of experience in care management or a similar role.
- Deep understanding of the unique needs and challenges facing older adults and individuals with disabilities.
- Exceptional communication, organizational, and problem-solving skills.
- Self-motivated with the ability to work independently and as part of a team.
- Competency with electronic health records and relevant technological tools.
