Job Summary

We are seeking a skilled Speech Therapist to join our Home Care team in Bucks County. As a Speech Therapist, you will provide one-to-one care to patients in their homes, utilizing your expertise to restore function, relieve pain, and prevent disability following illness, disease, injury, or surgery.

Key Responsibilities
  • Consult, evaluate, plan, and administer skilled Speech Therapy services prescribed by a physician to homebound patients.
  • Collaborate with an interdisciplinary team to ensure patient needs are met and quality of care is achieved.
  • Manage, oversee, and provide primary patient care delivery to a select group of patients, supporting the patient-centered care model.
  • Interact with internal and external care group members as needed.
Requirements
  • 1 year of clinical practice as a licensed Speech Therapist required.
  • Currently licensed to practice as a Speech Therapist in PA.
  • Working knowledge in navigating through Excel and utilization of Microsoft Outlook and Word.
  • Excellent communication and problem-solving skills.
  • Excellent organization and time management skills.
  • Ability to multi-task.
  • Excellent interpersonal skills, and ability to work with an Interdisciplinary Team approach.
  • Self-confident and assertive, flexible, reliable, and dependable.
  • Positive, can-do attitude.
  • Currently licensed or eligible for a license to operate an automobile in the applicable state.
  • CPR certification.
  • Regular availability to work.
  • Recent home care experience preferred.
  • Valid Driver's license in PA.
