Job Title:
Chronic Registered Nurse

Job Summary:

The Chronic Registered Nurse will work with patients to develop personalized care plans and provide ongoing support to manage chronic respiratory conditions.

As a key member of the care team, the nurse will collaborate with healthcare professionals to ensure seamless care coordination and optimal patient outcomes.


Responsibilities:

  • Develop and implement comprehensive care plans for patients with chronic respiratory conditions
  • Provide education and coaching on disease management and self-care techniques
  • Coordinate with healthcare providers to ensure timely and effective care
  • Monitor patient progress and adjust care plans as needed
  • Communicate effectively with patients and families to ensure understanding and compliance
  • Collaborate with multidisciplinary teams to address complex patient needs
  • Stay up-to-date with the latest research and best practices in chronic care management
  • Document patient interactions and care plans in a timely and accurate manner
  • Participate in quality improvement initiatives to enhance patient care and outcomes

Requirements:

  • Active and unrestricted Georgia RN license
  • At least three years of experience in adult health
  • Background in adult chronic health conditions (pulmonology, cardiology, care management)
  • Electronic medical record experience
  • Basic computer skills and proficiency in problem-solving and multitasking
  • Excellent communication and teamwork skills
  • Clean background check and drug screening
