About the Role

CenterWell Senior Primary Care is seeking a highly skilled and compassionate Nurse Practitioner to join our team in Lakewood, Georgia. As a key member of our integrated care team, you will provide proactive, preventive care to seniors, including wellness visits, physical exams, chronic condition management, screenings, and minor injury treatment.

Responsibilities
  • Provide high-quality, patient-centered care to seniors in their homes, focusing on personalized experiences and addressing the factors that impact patient well-being.
  • Work collaboratively with our integrated care teams, including physicians, nurses, behavioral health specialists, and other healthcare professionals to deliver comprehensive care.
  • Conduct thorough assessments, develop and implement individualized care plans, and provide ongoing monitoring and support to patients.
  • Prescribe medications, order diagnostic tests, and refer patients to specialists as needed.
  • Stay up-to-date with the latest clinical guidelines, research, and best practices in primary care and gerontology.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote excellent patient care.
Requirements
  • Valid state driver's license, clean driving record, and proof of auto insurance.
  • Bachelor's degree in nursing or a related field.
  • Minimum 2 years of clinical experience as a Nurse Practitioner.
  • Active, unrestricted state license and DEA license.
  • Board certification as a Nurse Practitioner.
  • Senior patient care experience and strong technical skills.
  • 2 or more years of project leadership experience.
  • Active BLS through the AHA or ARC.
  • Strong belief in the revolutionary nature of house calls and passion for improving care delivery.
  • Willingness to work in a dynamic environment that requires last-minute problem-solving and out-of-the-box thinking.
Preferred Qualifications
  • Master's degree in nursing or a related field.
  • House call experience.
  • Urgent care, primary care, family practice, or internal medicine experience.
  • Value-based care knowledge and/or experience.
  • HEDIS quality metrics knowledge and/or experience.
