About YouWe are looking for a Primary Care Provider with extensive clinical experience in complex care and a passion for improving health outcomes and patient experience. You will be comfortable with broad spectrum primary care and eager to care for patients in the modality that makes the most sense for them. You will be driven to improve systems that enable personalized, empathetic care for individuals and families in the community.
Responsibilities- Act as a key primary care practitioner on our clinical team
- Engage in team-based care with a virtual team of admin, care navigators, PCPs, and clinical specialists
- Deliver primary care in a patient-centric and provider-centric model combining virtual and in-person care
- Drive ongoing improvements for our care model, optimizing outcomes while decreasing inefficiency
- 5+ years of clinical experience in complex care, chronic care, and primary care
- Innovative and creative leadership qualities
- Proven experience with successful behavior modification
- Comfort with basic procedural skills
- Completed an accredited NP program with certification to practice
- Active licensure in Georgia or ability to obtain license by start date with Galileo Co.
- For the safety of our patients and staff, employees in this role must be fully vaccinated with all necessary vaccinations.
The annualized salary range for 40 hours/week is $148,000-$160,000. Depending on your schedule of 1 or 2 days per week, the range will be between $29,600-$64,000.
